Obituary of Phillip Polk

Phillip Wayne Polk, 61, passed away on July 14, 2023. Born in Louisville on July 8, 1962, he was the son of the late Clyde Polk, Jr. and Ozella Ashby Gebo, and is survived by his daughters Crystal Polk, Amber Polk, and Katie Polk.
A memorial service will be held at the Bardstown United Methodist Church Second Campus, 5100 Louisville Road, Bardstown, KY on Saturday, July 29th at 5:00 pm. Visitation will begin at 4:00 pm.
